Fee-for-service

Fee-for-service is where a health care provider charges for each service given a patient. This contrasts with capitation, where the insurer pays the providers a fixed monthly amount for each patient regardless of services given.

Both approaches require insured patients to pay a portion of the fees, often as co-payments, and the insurance company pays the rest, often at a large discount.

Uninsured persons are charged the entire fee.
